Description
Posterior cervical anatomy is centered on the occipital vein as it descends from the scalp over the occiput. The vessel courses inferiorly in the midline and paramedian subcutaneous plane, superficial to the splenius capitis and upper fibers of trapezius, then turns anteroinferiorly toward the deep venous channels at the posterior triangle. Along its path it lies near the superior nuchal line and external occipital protuberance, with smaller venules converging from the posterior scalp and nuchal region. Emphasis on the occipital vein matters because its drainage pattern is variable, commonly communicating with the suboccipital venous plexus and then with the vertebral and deep cervical veins, or tracking toward the internal jugular vein via connections to the posterior auricular and external jugular system. That variability becomes relevant during posterior scalp laceration management, suboccipital approaches, and posterior cervical dissections, where brisk venous bleeding can arise from these emissary-type connections and from the plexiform veins around the atlanto-occipital region. Surgical planes are easier to teach here. You can see exactly where a superficial vessel becomes a deep problem.
