Description
Running along the medial border of the male foot, the medial marginal vein courses superficial to the abductor hallucis and adjacent to the medial aspect of the calcaneus and navicular, beginning near the venous plexus of the hallux and medial forefoot. Proximally it ascends anterior to the medial malleolus, where it continues as the great saphenous vein along the medial leg. Deep to this superficial channel, the tibia and fibula frame the distal leg, with tarsal and metatarsal bones forming the medial longitudinal arch and providing palpable bony landmarks for surface anatomy. Arterial counterparts in the region logically include branches of the dorsalis pedis and the medial plantar artery supplying the medial forefoot. A medial perspective clarifies the relationship between the medial marginal vein and the saphenous opening route used for venous access and bypass conduit harvesting. It also supports teaching of varicose vein patterns and reflux that often tracks the great saphenous system, with incompetent perforators in the medial lower leg producing visible tortuosity and edema along this same line.
