Description
Rendered in lateral profile on an adult male, the long head of the biceps femoris is isolated against the posterior thigh, extending from the ischial tuberosity of the pelvis proximally to the lateral aspect of the knee distally. Its fusiform belly lies posterior to the femoral shaft and tracks inferiorly toward the fibular head, where it joins the short head to form the lateral hamstring tendon. Anterior to this posterior compartment pathway, the femur, patella, tibia, and fibula provide clear bony reference points for orientation from hip to ankle. Landmarks matter. Clinically, the long head is the hamstring most often injured at the proximal myotendinous junction during sprinting and kicking, and its ischial origin helps distinguish a true proximal hamstring avulsion from referred posterior thigh pain. The lateral view makes the relationship to the posterolateral corner of the knee easy to teach, including how the biceps femoris tendon can contribute to lateral knee pain or snapping and how traction on the common fibular (peroneal) nerve near the fibular neck becomes relevant during repair or after high-grade strain.
