Description
Prominent along the posterior thoracic and upper lumbar trunk, the longissimus thoracis is rendered as the broad intermediate column of the erector spinae, positioned lateral to spinalis and medial to iliocostalis. Its muscle belly runs parallel to the spinous processes, spanning from the thoracolumbar fascia and lumbar transverse processes toward the thoracic transverse processes and adjacent rib angles. Medially, the laminae and spinous processes form the axial landmark line, while laterally the ribs arc anterolaterally beneath the muscle’s segmental slips. Scapulae and proximal humeri frame the superior field, and the iliac crests and posterior pelvis define the inferior boundary. Clear midline orientation. Longissimus thoracis matters because it is a frequent generator of mechanical mid back and thoracolumbar pain, where overuse, poor endurance, or acute strain produces focal tenderness and myofascial trigger points adjacent to the transverse processes and rib angles. The posterior view clarifies why thoracic paraspinal spasm can restrict coupled extension and ipsilateral rotation, and it also helps when planning posterior approaches to the thoracic spine, where splitting the erector spinae along natural intermuscular planes reduces unnecessary denervation. Segmental dorsal rami innervation and proximity to the costotransverse joints make this region relevant when interpreting pain referral after facet arthropathy or costovertebral dysfunction. Use this illustration in gross anatomy and kinesiology teaching to differentiate the erector spinae columns and to cue palpation landmarks during surface anatomy labs.
