Description
Posterior thoracic anatomy is presented on a standing male child, with the lungs rendered semi-transparently against the back so the dorsal (costal) surfaces and fissural anatomy read clearly. On the right, the superior, middle, and inferior lobes are separated by the oblique and horizontal fissures, while the left lung shows superior and inferior lobes divided by a single oblique fissure. The trachea descends in the midline and bifurcates into the right and left main bronchi at the carina, with the hila positioned medial to the lung fields adjacent to the vertebral column. Lobes sit lateral to the posterior mediastinum and extend inferiorly toward the diaphragm, the right base typically projecting slightly lower than the left. A posterior perspective matters in pediatrics because auscultation and percussion are performed predominantly over the back, where lung parenchyma is least obscured by the heart and anterior mediastinum. This view supports teaching of lobar localization for common childhood disease, for example right middle lobe pneumonia that can be missed posteriorly versus lower lobe consolidation that often presents with posterior basal crackles, as well as the typical distribution of atelectasis in dependent posterior segments during sedation or postoperative recovery. It also maps the airway axis from trachea to main bronchi, a practical reference when explaining why aspirated foreign bodies more often lodge in the right bronchial tree.
