Description
Semi-transparent anterior thoracic soft tissues reveal the lungs of an elderly white male seated within the rib cage, with the apices extending superior to the level of the first ribs and the bases resting on the diaphragm. The trachea descends in the midline and bifurcates at the carina into right and left main bronchi, which enter the hila on the medial surfaces of each lung. Right lung volume appears greater, consistent with its three-lobed configuration, while the left lung sits slightly more medial and inferior near the cardiac silhouette, with less parenchymal breadth. Portions of the upper abdominal viscera are faintly visible inferior to the costal margin, reinforcing the thoracoabdominal relationship. Age context matters. In an elderly patient, reduced elastic recoil and common parenchymal changes such as emphysematous hyperinflation or basal atelectasis alter the expected position of the diaphragmatic domes and the apparent aerated lung volume within the bony thorax.
