Description
Centered on the anterior orbit, the orbital septum is rendered as a thin fibrous lamina spanning from the orbital rim toward the eyelids, forming a curtain between the superficial palpebral tissues and the deeper orbital fat. Superiorly it relates to the supraorbital margin of the frontal bone, and inferiorly to the infraorbital margin of the maxilla and zygomatic bone, with the medial boundary approaching the anterior lacrimal crest. Surrounding bony landmarks of the male skull are visible, including the nasal bones, the orbital aperture, and the optic canal at the orbital apex, with maxillary and mandibular dentition providing inferior facial reference. Surgeons care about the orbital septum because it defines what is preseptal versus postseptal in eyelid infection and trauma, a distinction that changes urgency and management when orbital cellulitis is suspected. It also acts as a restraint to anterior prolapse of orbital fat, and its relationship to the levator aponeurosis and tarsal plate explains why improper dissection during upper blepharoplasty or ptosis repair can lead to lid retraction, hematoma, or visible fat herniation.
