Description
Posterior shoulder girdle anatomy is presented from a dorsal perspective in a male, centering on the rhomboid minor as it spans from the nuchal ligament and spinous processes of C7 to T1 to the medial border of the scapula at the level of the scapular spine. Superiorly, the upper trapezius forms the superficial roof over the cervicothoracic junction, while the levator scapulae descends along the lateral neck to the superior angle of the scapula. Inferior to rhomboid minor, the larger rhomboid major occupies the medial scapular border, and laterally the scapular spine and acromial region provide bony landmarks for orientation. Fiber direction is clear. This posterior view matters when teaching scapulothoracic mechanics, because rhomboid minor works with rhomboid major to retract and downwardly rotate the scapula, counterbalancing the protraction forces of serratus anterior and pectoralis minor. Clinically, focal tenderness along the medial scapular border and pain with resisted retraction often reflects rhomboid strain or myofascial trigger points, and the illustration supports discussions of dorsal scapular nerve involvement (C5), including traction injuries or entrapment near the middle scalene that can weaken rhomboids and alter scapular posture.
