Description
Running from the nuchal ligament and spinous processes of C7 to T1 to the medial end of the scapular spine, the rhomboid minor sits deep to the trapezius on the posterior thorax. Medial to its lateral scapular attachment, the muscle lies immediately lateral to the thoracic spinous processes and the erector spinae mass, with the rhomboid major continuing inferiorly along the medial border of the scapula. Superiorly, levator scapulae approaches the superomedial angle of the scapula, while latissimus dorsi occupies the lower back more inferolaterally, framing the upper back region around the shoulder girdle. Orientation matters here because the rhomboid minor is easiest to understand in relation to scapular retraction and downward rotation, movements that often become asymmetrically limited in patients with periscapular pain and rounded-shoulder posture. The dorsal scapular nerve (C5) and dorsal scapular artery travel on the deep surface of the rhomboids near the medial scapular border, a relationship that helps explain focal tenderness and referred discomfort when the muscle is strained or when the nerve is irritated in athletes performing repetitive pulling. The medial scapular spine and adjacent interspinous region guide palpation and injection planning when differentiating rhomboid pain from trapezius trigger points or cervical radiculopathy.
