Description
Centered on the posterior thorax, the rhomboid major is highlighted spanning from the midline spinous processes of T2 to T5 to the medial (vertebral) border of the scapula, inferior to the scapular spine. Along its superior margin, the smaller rhomboid minor lies closer to the root of the scapular spine and the lower cervical to upper thoracic spinous processes. Deep to the superficial trapezius, both rhomboids sit medial to the scapulae and lateral to the thoracic vertebral column, with the ribs forming the lateral background of the chest wall. Clear bony landmarks. Clinically, this posterior relationship matters because the rhomboids are common pain generators in interscapular myofascial syndromes and are frequently implicated in scapular dyskinesis, where weakness or inhibition allows excessive scapular protraction and anterior tilt. Their position deep to trapezius also explains why tenderness can be difficult to localize on exam, and why trigger point referral is often described along the medial scapular border rather than the muscle belly itself.
