Description
Posterior surface anatomy of the male upper back is rendered with the skin removed to expose the superficial and intermediate scapulothoracic musculature. The rhomboid major lies deep to trapezius, spanning from the spinous processes of approximately T2 to T5 to the medial border of the scapula, inferior to the scapular spine and superior to the inferior angle. Inferolaterally, latissimus dorsi sweeps toward the humerus, while erector spinae columns sit paramedian along the thoracolumbar fascia. Deltoid caps the lateral shoulder, and triceps brachii occupies the posterior arm; inferiorly the posterior chain continues into gluteus maximus, hamstrings, and gastrocnemius-soleus. Color emphasis on the lower trapezius is a useful cue because this slip overlies the rhomboids and often confuses learners when they try to separate scapular retraction from scapular depression during physical exam. Rhomboid major retracts and downwardly rotates the scapula, and weakness from dorsal scapular nerve palsy (often traction-related in the posterior triangle or from repetitive overhead load) produces subtle lateral drift of the medial scapular border rather than the pronounced winging typical of long thoracic nerve injury. Small details matter here. This posterior perspective also maps cleanly onto common surgical corridors, including the posterior approach to the scapula and the muscular planes encountered when accessing the medial scapular border.
