Description
Posteriorly, the superficial trapezius is reflected or removed to expose the rhomboid minor and rhomboid major spanning from the midline thoracic spine to the medial border of the scapula. Rhomboid minor lies superior to rhomboid major, typically running from the spinous processes around C7 to T1 to the scapular root of the spine, while rhomboid major descends from approximately T2 to T5 to insert along the medial scapular border inferior to the spine. Medial to these muscles sit the thoracic spinous processes, and laterally the scapular bodies frame the muscle bellies as they course inferolaterally from vertebrae to scapula. Functionally, this posterior view clarifies how the rhomboids retract the scapula and contribute to downward rotation by pulling the medial border toward the vertebral column, actions that oppose serratus anterior during protraction. This relationship matters in scapular dyskinesis, where weakness or inhibition of the rhomboids (often paired with lower trapezius imbalance) contributes to medial border winging and altered scapulohumeral rhythm during overhead activity.
