Description

Centered on the posterior thorax of an adult male, the rhomboid minor and rhomboid major span from the midline spinous processes to the medial border of the scapula. Rhomboid minor runs from the nuchal ligament and C7 to T1 spinous processes to the root of the scapular spine, lying superior to rhomboid major, which descends to insert along the medial scapular border toward the inferior angle. Superficial to both, trapezius forms a broad sheet from the external occipital protuberance and nuchal ligament down to the lower thoracic spinous processes, with fibers inserting on the clavicle, acromion, and scapular spine. Bony context includes the cervical, thoracic, and lumbar vertebrae, posterior ribs, scapulae, proximal humeri, and pelvis. Rhomboids are the workhorse retractors of the scapula, and their relationship to trapezius is the teaching point in a posterior dissection: trapezius must be reflected to expose the rhomboid plane and the dorsal scapular nerve (C5) that courses deep along the medial scapular border with the dorsal scapular artery. Weakness from dorsal scapular neuropathy can present as medial scapular pain, difficulty with retraction, and subtle scapular winging that differs from the classic long thoracic nerve pattern.
