Description
Running along the anterolateral aspect of the lumbar spine, the paired psoas major muscles are highlighted against the bony landmarks of the vertebral column, ribs, and pelvis in a male anterior view. Each muscle arises from the lateral surfaces of T12 to L5 vertebral bodies and intervertebral discs, with slips from the lumbar transverse processes, then descends inferiorly across the pelvic brim toward the lesser trochanter of the femur. Medial to psoas major sits the lumbar vertebral column, while the iliacus occupies the iliac fossa laterally and joins psoas as the iliopsoas tendon. When present, psoas minor lies anterior to psoas major and fades into the iliopubic eminence. Clear, bilateral symmetry. Clinically, the anterior perspective clarifies why the psoas is both a hip flexor and a lumbar stabilizer, and why pain can refer to the groin or anterior thigh when the muscle or its fascial compartment is irritated. This is also the corridor for a psoas abscess, often tracking from vertebral osteomyelitis or gastrointestinal sources, and it explains the flexed-hip posture many patients adopt to reduce stretch and discomfort. Surgeons and interventionalists will recognize the psoas as a key landmark for lateral lumbar approaches and for understanding relationships near the pelvic brim and hip joint.
