Description
Running anterior to the lumbar vertebral bodies, the psoas major appears as a paired, fusiform muscle arising from the sides of T12 to L5 and their intervening discs, then descending inferolaterally toward the pelvic brim. Each belly hugs the anterolateral lumbar spine, with the medial margin close to the vertebral column and the lateral margin approaching the iliac fossa as it heads toward the lesser trochanter of the femur via the iliopsoas tendon. Superiorly, the lower ribs frame the upper attachment area, while inferiorly the muscle passes anterior to the hip joint as it enters the proximal thigh. Bony landmarks include the lumbar vertebrae, sacrum, and the hip bones (ilium, ischium, and pubis) forming the pelvis. Anterior visualization of psoas major matters because its deep course sets the stage for common clinical problems that are otherwise hard to conceptualize: psoas abscess tracking along the muscle from the lumbar region into the groin, or iliopsoas tendinopathy and bursitis presenting as anterior hip pain with snapping. The muscle also provides a key corridor for retroperitoneal and lateral lumbar spine approaches, where surgeons work near the lumbar plexus embedded within psoas major. Orientation is everything here. Educators can drop this figure straight into gross anatomy and musculoskeletal modules to teach hip flexion mechanics and the relationship between the lumbar spine, pelvis, and femur, and it also fits cleanly into surgical anatomy discussions of transpsoas (lateral lumbar interbody fusion) access.
