Description
Arising from the anterolateral surfaces of the lumbar vertebral bodies and intervertebral discs, the paired psoas major muscles descend inferiorly along the pelvic brim toward the proximal femur, where each tendon inserts on the lesser trochanter. From an anterior perspective, their bulky bellies sit medial to the iliacus within the iliopsoas complex, flanking the lumbar spine on the right and left while remaining anterior to the transverse processes. Superiorly, the lower ribs and thoracolumbar junction frame the upper origin, and inferiorly the iliac bones and femoral shafts establish the hip relationship. Clear midline landmarks. Clinically, this is the corridor used for anterior approaches to the lumbar spine (including oblique lateral interbody fusion), where splitting or mobilizing the psoas exposes the disc space but places the lumbar plexus at risk within the muscle belly. Psoas spasm can mimic hip pathology, and enlargement or fluid within the psoas (psoas abscess or hematoma) follows the same fascial compartment, often presenting with pain on hip extension and a flexed posture. The insertion on the lesser trochanter also explains why resisted hip flexion can reproduce symptoms in iliopsoas tendinopathy or internal snapping hip. Use this asset for teaching iliopsoas anatomy in gross anatomy and musculoskeletal courses, for illustrating anterior lumbar surgical planning in spine texts, and for patient education materials discussing hip flexor strain versus lumbar-origin pain.
