Description
Occupying the iliac fossa on each side, the iliacus appears as a broad triangular sheet arising from the internal surface of the ilium and descending inferomedially toward the pelvic brim. Medial to it, the psoas major is suggested along the lumbar spine, and the two converge inferiorly to form the iliopsoas tendon as it courses toward the lesser trochanter of the femur, anterior to the hip joint capsule. Bony landmarks frame the muscle belly, including the iliac crest superiorly, the acetabulum laterally, and the sacrum in the midline posteriorly. Hip articulation is clear. Anterior exposure of the iliacus matters because it sits in the same corridor used for common anterior hip and pelvic approaches, where distinguishing muscle belly, iliopsoas tendon, and acetabular rim reduces confusion and iatrogenic injury. Tightness or tendinopathy of the iliopsoas can produce anterior groin pain and an audible snapping hip as the tendon moves across the iliopectineal eminence and femoral head, a pattern often evaluated clinically and on ultrasound-guided injection. Hematoma within the iliopsoas compartment, including the iliacus, is also a recognized cause of femoral neuropathy with anticoagulation. Think femoral triangle referral patterns.
