Arising from the iliac fossa on the internal surface of the ilium, the iliacus occupies the concavity of the pelvis and converges inferiorly toward the anterior hip, lying lateral to the psoas major as both descend to form the iliopsoas. Superiorly, the lower lumbar vertebrae with their intervertebral discs frame the origin region of psoas major, while the pelvic ring (ilium, ischium, and pubis) defines the acetabulum medial to the proximal femur. Laterally on the outer ilium, gluteus medius and gluteus minimus layer over the lateral hip, separated from the iliacus by the iliac wing. The femoral head sits deep in the acetabular socket. For teaching hip flexion mechanics, this configuration matters because iliacus and psoas major share a distal tendon and line of pull but have different proximal attachments, a distinction that clarifies why iliopsoas dysfunction can present as anterior groin pain rather than lateral hip pain typical of gluteus medius tendinopathy.
