Description
Posterior trunk anatomy is presented with the scapular girdle centered on the rhomboid minor, a small quadrangular muscle spanning from the nuchal ligament and spinous processes of C7 to T1 to the medial border of the scapula at the level of the scapular spine. It lies inferior to the levator scapulae and superior to the rhomboid major, with its fibers running inferolaterally from the vertebral column toward the scapula. Superficially, the trapezius blankets the region from the midline to the acromion and scapular spine, and the deltoid caps the lateral shoulder while the latissimus dorsi appears more inferior and lateral. Clinically, this posterior view helps clarify why scapular retraction and downward rotation depend on coordinated action between the rhomboid minor, rhomboid major, and middle trapezius, and why imbalance often presents as medial scapular border prominence and altered scapulothoracic rhythm. Dorsal scapular nerve (C5) irritation or injury can weaken the rhomboids and levator scapulae, producing fatigue with sustained posture and pain along the medial scapular border that can mimic cervical radiculopathy.
