Description
Emerging from the femoral nerve in the mid-thigh, the saphenous nerve is traced distally as it courses through the adductor canal (subsartorial canal) with the femoral artery and vein, then becomes superficial near the medial knee. From a posterior perspective of the lower limb, its cutaneous territory is logically related to the medial leg and medial malleolar region, lying medial to the posterior compartment musculature, including gastrocnemius and soleus, and adjacent to the tibia’s medial border. Larger posterior neural structures are also apparent as context, with the tibial nerve descending in the posterior compartment and the common fibular nerve diverging laterally near the fibular head. Bony landmarks such as the tibia, fibula, and calcaneus anchor the relationships. A posterior view is useful because it forces the reader to reconcile a primarily anterior compartment origin (femoral nerve, adductor canal) with symptoms that present around the medial knee and along the medial leg, where the saphenous nerve is often injured or entrapped. Sensory loss or dysesthesia along the medial tibia after knee arthroscopy, medial meniscal surgery, or hamstring tendon harvest commonly reflects irritation of the infrapatellar branch of the saphenous nerve, while catheter placement and local anesthetic spread in an adductor canal block target the same parent trunk.
