Description
Arising from the parotid plexus anterior to the auricle, the buccal branch of the facial nerve (CN VII) courses anteriorly across the lateral face, running superficial to the masseter and toward the buccinator and orbicularis oris at the level of the upper lip and nasal ala. Superiorly, temporal and zygomatic branches sweep toward the lateral orbital region, framing the lateral canthus and zygomatic arch. Fine motor fibers fan out across the cheek in a largely anteromedial direction, where they intersect the facial artery and vein as they ascend toward the nasolabial fold. Small connections with the buccal nerve (V3) are implied in this territory. Delicate anatomy. Damage to these fibers produces selective weakness of midfacial expression, classically affecting smiling, upper lip elevation, and cheek tension during blowing or whistling, even when frontal function appears preserved. Because the buccal branches run in a shallow plane over the masseter before entering the mimetic musculature, this view maps the danger zone for iatrogenic injury during parotidectomy, facelift (rhytidectomy), and midface approaches, and it helps explain why lacerations near the anterior border of the parotid gland can yield asymmetric movement around the oral commissure. The relationship to the parotid region also matters when evaluating facial nerve palsy patterns and localizing lesions relative to the stylomastoid foramen and parotid plexus.
