Description
Arising from the parotid plexus within the parotid gland, the buccal branch of the facial nerve (CN VII) courses anteriorly across the masseter, then fans medially toward the buccinator and orbicularis oris in the mid-face. A lateral, slightly oblique head-and-neck perspective places the parotid region posterior and lateral to the ramus of the mandible, with the zygomatic area superior to the oral commissure and the cheek musculature layered over the maxilla. Superficial facial muscles and deeper muscles of mastication, including temporalis superiorly and masseter laterally, provide the key spatial landmarks. Nerve fibers are rendered as fine yellow ramifying lines against red muscle bellies. Facial nerve topography at the anterior parotid border is where most iatrogenic injuries occur, and the buccal branch is a frequent culprit during parotidectomy, facelift (rhytidectomy), and masseteric approaches. Weakness of the buccinator and upper lip elevators can present as food trapping in the vestibule, an asymmetric smile, or impaired lip seal, findings that help localize lesions distal to the stylomastoid foramen yet proximal to terminal midface arborization. Cross-innervation with adjacent zygomatic and marginal mandibular branches varies, so a clear graphic of the fanned distribution supports realistic teaching and operative planning. Small fibers matter.
