Description
Arising from the facial nerve after it exits the stylomastoid foramen and traverses the substance of the parotid gland, the zygomatic branch courses anteriorly and superiorly across the lateral face toward the zygomatic bone and lateral canthus. Fine yellow nerve fibers radiate superficially over the malar region, running lateral to the nasolabial fold and superior to the oral commissure as they approach orbicularis oculi. Red facial muscles form the primary backdrop, with orbicularis oculi encircling the palpebral fissure and adjacent elevators of the upper lip and zygomaticus muscles occupying the cheek. Red arteries and blue veins track in parallel within the subcutaneous plane, reinforcing the crowded neurovascular corridor over the zygoma. A tight neighborhood. Because the zygomatic branch supplies motor innervation to the periorbital musculature, it is a key structure in eyelid closure and blink, and its injury can present as lagophthalmos with exposure keratopathy. Surgeons planning facelift dissections, parotidectomy, temporomandibular or zygomatic arch approaches, or repair of cheek lacerations use this topography to avoid iatrogenic palsy, especially where the branch becomes superficial near the anterior border of the parotid gland and over the zygomatic eminence. This view also supports teaching facial nerve branching variability and expected cross-innervation with the buccal branch, an anatomic reason partial deficits can look deceptively mild. Ideal applications include head and neck anatomy teaching in medical and dental curricula, operative atlases illustrating safe planes for rhytidectomy and parotid surgery, and patient education graphics explaining facial nerve weakness after trauma or tumor resection.
