Description
Arising from the cervical plexus at the C2 to C3 ventral rami, the transversus colli nerve (transverse cervical nerve) courses anteriorly across the lateral neck, emerging near the posterior border of the sternocleidomastoid before crossing its superficial surface. Superiorly it approaches the submandibular region and inferior margin of the mandible, distributing cutaneous branches over the anterior cervical triangle while remaining superficial to the deep cervical fascia. Nearby relationships commonly visible in this field include the platysma, sternocleidomastoid, and the inferior border of the mandible, with superficial veins tracking in the same plane. Surface anatomy matters here. For neck dissection and regional anesthesia, this nerve is one of the key superficial sensory branches targeted in a superficial cervical plexus block, where local anesthetic is placed along the posterior border of the sternocleidomastoid to cover anterior neck skin. Surgeons also watch for it during carotid endarterectomy exposure and submandibular gland approaches, since traction or transection can leave a patch of numbness or dysesthesia over the anterolateral neck. Its close proximity to the external jugular vein and the great auricular nerve makes this region a frequent source of identification errors in teaching and in the operating room.
