Description
Presented from an anterior perspective of the male head and neck with skin removed, the facial nerve (cranial nerve VII) is traced as it emerges from the parotid region and sends its marginal mandibular branch forward along the inferior border of the mandible. That slender motor branch courses superficial to the facial vessels near the anteroinferior masseter, then turns medially toward the depressor anguli oris and depressor labii inferioris, with the platysma forming a superficial sheet in the upper neck. Adjacent muscles of expression, including orbicularis oris, zygomaticus major and minor, buccinator, and orbicularis oculi, provide clear orientation to the perioral and midface territories supplied by CN VII. Key landmarks read well. Surgical relevance centers on how variable the marginal mandibular nerve can be as it crosses the mandible and submandibular triangle, sometimes dipping 1 to 2 cm below the mandibular border before rising toward the lower lip depressors. Injury during submandibular gland excision, neck dissection, mandibular angle approaches, or even a low incision for a rhytidectomy produces the classic asymmetric smile, with lower lip depression and eversion weakened on the affected side while sensation remains intact. The anterior view clarifies the difference between CN VII motor branches and trigeminal (CN V) sensory distribution, a distinction that drives bedside cranial nerve localization.
