Description
Arising at the stylomastoid foramen just posterior to the mandibular ramus, the facial nerve (cranial nerve VII) fans anteriorly within the parotid region into temporofacial and cervicofacial divisions, then into the temporal, zygomatic, buccal, marginal mandibular, and cervical branches across the male face. Superficial muscles of expression form the primary field of innervation, including frontalis superiorly, orbicularis oculi around the palpebral fissure, zygomaticus major and minor coursing inferomedially to the oral commissure, and orbicularis oris encircling the mouth. Lateral to the zygomatic arch, temporalis and masseter are shown as prominent landmarks, while platysma spreads thinly over the anterolateral neck. A small but clinically recognizable companion, the chorda tympani, is included as a branch of VII that joins the lingual nerve in the infratemporal fossa for taste to the anterior two-thirds of the tongue and parasympathetic supply to submandibular and sublingual glands. Anterior mapping of CN VII matters because most iatrogenic injuries occur after the nerve has divided within the parotid gland, where the branching pattern varies but consistently crosses key surgical corridors. Parotidectomy, facelift rhytidectomy dissection, and repair of lacerations along the mandibular border all risk the marginal mandibular branch, with postoperative asymmetry from depressor anguli oris weakness. Bell palsy and Ramsay Hunt syndrome are also taught best against this branching schema, because the pattern of motor loss helps localize lesions proximal or distal to chorda tympani involvement. A critical landmark.
