Description
Running in a lateral profile of the male head and neck, the facial nerve (cranial nerve VII) is traced from its deep course in the temporal bone to its extracranial arborization within the parotid region. After exiting the skull at the stylomastoid foramen, the main trunk passes anterolaterally, then divides within the parotid gland into the temporofacial and cervicofacial divisions, giving rise to temporal, zygomatic, buccal, marginal mandibular, and cervical branches that spread anteriorly across the masseter toward the muscles of facial expression. Proximal branches such as the posterior auricular nerve and the nerve to the posterior belly of digastric and stylohyoid sit posterior and inferior to the ear region, while the chorda tympani is indicated as a preganglionic parasympathetic and special sensory branch arising proximally, coursing toward the infratemporal fossa to join the lingual nerve. For operative anatomy, this lateral relationship is the one surgeons think in. Parotidectomy and facelift (rhytidectomy) dissections hinge on identifying the facial nerve trunk near the tragal pointer, the tympanomastoid suture, and the posterior belly of digastric, then protecting the branching pattern as it crosses superficial to the masseter. Bell palsy and temporal bone trauma also make more sense when you can follow VII from its intratemporal segment to its superficial distribution. Landmarks matter.
