Description
Arising at the pontomedullary sulcus, the facial nerve (CN VII) courses anteriorly alongside the nervus intermedius, then travels toward the cerebellopontine angle before entering the internal acoustic meatus with the vestibulocochlear nerve (CN VIII). In an anterior orientation, the brainstem landmarks of the pons and medulla frame the root entry and exit zone, and the intermediate portion is distinguished from the motor root as it converges laterally. Inferior and anterior to this cranial base pathway, the tongue musculature occupies the midline, with branching neural elements extending into intrinsic and extrinsic muscles. Small-caliber vascular channels accompany these nerves along the floor of the mouth. Separating the facial nerve proper from the intermediate nerve matters in practice because the nervus intermedius carries taste fibers from the anterior two thirds of the tongue (via chorda tympani) and parasympathetic outflow to the lacrimal and salivary glands, while the motor root supplies muscles of facial expression after its intratemporal course. A compact anterior view that ties the pontomedullary origin to oral targets clarifies why lesions at the cerebellopontine angle, such as vestibular schwannoma, can produce facial weakness with altered taste or reduced lacrimation.
