Description
Arising from the thoracic and lower cervical spinous processes, the semispinalis cervicis is brought into focus in a posterior female neck with the superficial trapezius opened to expose the deep extensor layer. Its fascicles ascend superomedially to attach on the cervical spinous processes, lying medial to the splenius capitis and splenius cervicis and deep to them where those muscles remain present. Lateral to this column of muscle, the levator scapulae descends from the upper cervical transverse processes toward the superior angle of the scapula, while rhomboid minor and major sit inferiorly along the medial border of the scapula beneath the trapezius. Fiber direction is clear. Semispinalis cervicis is a primary segmental extensor and stabilizer of the cervical spine, and this posterior dissection-style view is the one that best clarifies its relationship to the midline spinous processes where you palpate tenderness and where posterior midline approaches are planned. In whiplash-associated disorders and chronic cervicogenic headache patterns, deep cervical extensor dysfunction often persists even when the superficial trapezius dominates movement, so separating layers matters for teaching and for rehab cueing.
