Description
Centered in the posterior neck, the semispinalis cervicis is presented as a deep transversospinalis muscle lying anterior (deep) to the trapezius and typically deep to the splenius cervicis, with its fascicles running superomedially from upper thoracic transverse processes toward the cervical spinous processes. Superiorly it approaches the nuchal region beneath semispinalis capitis, while inferiorly it blends into the layered extensor mass over the upper thoracic spine. Laterally, longissimus cervicis and the levator scapulae region frame the deeper midline musculature, and the scapular contour and posterior deltoid anchor the shoulder girdle in the lower field. Depth is the point. Functionally, semispinalis cervicis drives cervical extension and contributes to contralateral rotation, and this posterior perspective clarifies why the muscle is often affected in acceleration-deceleration injuries and in postoperative neck pain after posterior cervical exposure. Detachment and prolonged retraction of the deep extensors during laminectomy or laminoplasty can produce denervation and atrophy of the dorsal ramus supplied musculature, so the layered relationship to trapezius and splenius matters when planning a midline approach and closure. Its segmental insertions on cervical spinous processes also make it a practical teaching reference for palpation landmarks and for understanding why midline tenderness can localize to specific cervical levels.
