Description
Running longitudinally along the posterior cervical spine, the longissimus cervicis is shown as the medial component of the erector spinae column, positioned lateral to the cervical spinous processes and medial to the more lateral longissimus capitis. Inferiorly it blends into the thoracic portion of longissimus, while superior fibers course toward the posterior tubercles of the transverse processes of C2 to C6. Adjacent landmarks in this posterior male view typically include the splenius capitis and splenius cervicis superficially, with the levator scapulae ascending along the superomedial border of the scapula toward the transverse processes of C1 to C4. Depth is the story. Seeing longissimus cervicis in posterior relationship to the cervical transverse processes matters when you are teaching or planning posterior cervical exposure, because this muscle forms part of the paraspinal envelope that is split, stripped, or retracted to reach the laminae and facet joints. It also helps explain why cervical extension and ipsilateral lateral flexion can be painful in paraspinal strain, and why targeted injections or dry needling near the transverse processes demand careful orientation to avoid drifting laterally toward the levator scapulae and the more superficial posterior triangle structures. The muscle’s segmental attachments make it a useful map for level counting during dissection.
