Posterior trunk anatomy is rendered with the abdominal viscera visualized through the integument, centering on the small intestine. Coiled loops of jejunum occupy the central and left abdomen, while the ileum descends inferiorly into the right lower quadrant, approaching the ileocecal junction; the duodenum sits more superiorly and to the right, curving around the pancreatic head before continuing as jejunum at the duodenojejunal flexure. From this back-facing perspective, the small bowel mass lies anterior to the lumbar vertebrae and posterior abdominal wall musculature, spreading inferiorly toward the pelvic brim. Mesenteric suspensory anatomy is implied by the fan-shaped distribution of the loops. Orientation is clear. This posterior view matters when teaching how abdominal symptoms can present as back pain and when correlating bowel position to retroperitoneal landmarks. Jejunal loops classically concentrate in the left upper quadrant while ileal loops favor the right lower quadrant, a pattern that helps clinicians interpret CT patterns of small-bowel obstruction, internal hernia, and Crohn disease distribution in the terminal ileum. It also supports discussions of referred pain pathways in midgut disease (T9 to T12) and why distention can be perceived deep in the flank or low back during ileus. Spatial logic drives diagnosis.
