Description

Running in the anterior neck, the sternothyroid muscle lies deep to the sternohyoid and medial to the sternocleidomastoid, spanning from the posterior manubrium and first costal cartilage to the oblique line of the thyroid cartilage. Superiorly it sits immediately inferior to the hyoid level muscles, while its cranial attachment places it adjacent to the laryngeal framework. Surrounding context in the head and upper torso logically includes the infrahyoid strap complex, surface facial musculature such as orbicularis oculi and orbicularis oris, and bony landmarks of the mandible, clavicles, sternum, and upper cervical vertebrae. Fiber direction is clear. For thyroid and laryngeal surgery, this relationship matters. The sternothyroid is routinely separated or partially divided to expose the thyroid gland and the laryngeal cartilages during thyroidectomy and tracheostomy, and its deep position explains why it functions more as a laryngeal depressor than a hyoid depressor in many dissection narratives.
