Description
Seen from a posterior, subcutaneous perspective on an adult male, the trapezius is rendered in red as the dominant superficial muscle spanning the posterior neck and upper thorax. Its superior fibers arise from the external occipital protuberance and medial superior nuchal line, blend with the ligamentum nuchae, then continue inferiorly from the spinous processes of C7 through T12. Laterally, the upper fibers course inferolaterally to the lateral third of the clavicle, the middle fibers run transversely to the acromion and spine of the scapula, and the lower fibers ascend superolaterally toward the medial end of the scapular spine, lying superficial to the levator scapulae and rhomboid major and minor. Fiber direction matters. The opposing pull of the upper and lower parts produces upward rotation of the scapula during overhead elevation, while the middle part retracts the scapula and stabilizes the shoulder girdle against the thoracic wall during pushing and lifting. Clinically, this anatomy frames classic findings in spinal accessory nerve (CN XI) injury after posterior triangle lymph node biopsy or neck dissection: ipsilateral shoulder droop, weak shoulder shrug, and impaired scapular upward rotation with fatigue and pain on arm abduction above 90 degrees. Clear margins between trapezius and the deltoid at the acromion also guide safe injection sites and posterior shoulder approaches. In teaching, this view supports surface anatomy labs and musculoskeletal courses when linking palpable landmarks, such as the nuchal midline and scapular spine, to the underlying trapezius. It also fits surgical and rehabilitation publications discussing accessory nerve palsy, scapular dyskinesis, trigger point mapping, and electromyography lead placement across upper, middle, and lower trapezius.
