Description
Posterior dorsal anatomy centers on the trapezius, highlighted as three fiber regions with superior fibers descending from the external occipital protuberance and nuchal ligament, middle fibers coursing transversely from the spinous processes, and inferior fibers ascending from the lower thoracic spinous processes. Laterally, the muscle inserts along the lateral third of the clavicle, the acromion, and the spine of the scapula, forming a broad superficial sheet over the upper back. Medially, its vertebral attachments align with the midline cervical and thoracic spine, while the right and left trapezius meet at the midline raphe. Beneath its margins you can infer the contours of levator scapulae superior-medially and rhomboid major and minor deep to the middle and inferior portions. Clear layering. For teaching scapulothoracic mechanics, this posterior perspective is the one you want because it makes the opposing force directions of the upper and lower trapezius obvious, and it frames how the middle trapezius contributes to scapular retraction and external rotation during overhead elevation. Clinically, it supports discussion of spinal accessory nerve (CN XI) injury after posterior triangle lymph node biopsy or neck dissection, where trapezius palsy produces shoulder droop, lateral scapular winging, and impaired upward rotation. Palpating the superior fibers at the posterolateral neck and tracking toward the acromion also helps anchor common myofascial trigger point patterns that refer pain to the occiput and temporal region.
