Description
Centered at the base of the brain, the Circle of Willis forms an arterial ring linking the internal carotid system to the vertebrobasilar circulation. The anterior cerebral arteries course anteromedially and meet through the anterior communicating artery, while each internal carotid artery gives rise laterally to the middle cerebral artery before continuing as the anterior cerebral. Posteriorly, the posterior cerebral arteries arise from the basilar artery and connect to the internal carotids via the posterior communicating arteries, creating an anastomosis around the optic chiasm and hypothalamic region. Variants such as hypoplastic posterior communicating arteries or an asymmetric A1 segment are often implied in this anatomy. Collateral flow is the teaching point. When an internal carotid artery stenoses, a patent anterior communicating artery can redistribute blood to the contralateral anterior cerebral territory, and posterior communicating arteries may supply anterior circulation from the vertebral system, a concept that explains watershed vulnerability and variable stroke patterns. A frequent clinical correlation is saccular (berry) aneurysm formation at branch points of the anterior communicating artery and posterior communicating artery, where hemodynamic stress concentrates and rupture can produce subarachnoid hemorrhage with a classic thunderclap presentation. Small, close quarters. Clinicians also associate a posterior communicating artery aneurysm with oculomotor nerve palsy due to its proximity to cranial nerve III. Neurology and neuroanatomy courses use this view to anchor cerebrovascular territories and to map common aneurysm sites for board-style questions. It also supports radiology and neurosurgery publications interpreting CTA, MRA, or catheter angiography of the anterior and posterior circulation, including preoperative planning for aneurysm clipping or endovascular coiling.
