Description
Viewed from the inferior aspect of the adult brain, the basal cerebral arteries form a ring-like anastomosis centered on the optic chiasm and the interpeduncular region. The internal carotid arteries give rise to the anterior cerebral arteries, linked across the midline by the anterior communicating artery, while the posterior communicating arteries course posteriorly to join the posterior cerebral arteries arising from the basilar artery at the superior border of the pons. Flanking this vascular circle, the optic nerves converge to the chiasm anteriorly, and the brainstem (midbrain, pons, medulla) continues inferiorly toward the foramen magnum, with the cerebellar hemispheres and vermis occupying the posterior fossa. Circle of Willis anatomy matters most when collateral flow is tested. Ruptured saccular aneurysms commonly occur at branch points within this arterial polygon, classically at the anterior communicating artery or the posterior communicating artery at the internal carotid junction, producing subarachnoid hemorrhage and, in the latter case, a third nerve palsy with ptosis and a blown pupil. Small. This inferior perspective also helps explain why basilar tip aneurysms or posterior circulation occlusion can impact thalamic and occipital perfusion through the posterior cerebral arteries. Teaching applications include neuroanatomy and cerebrovascular modules where students must connect cranial nerve landmarks to arterial relationships, as well as neurosurgery and neuroradiology content that pairs gross anatomy with CTA, MRA, or catheter angiography projections.
