Description
Arterial loops at the base of the brain frame the optic chiasm and pituitary stalk, with the internal carotid arteries ascending bilaterally to give rise to the anterior cerebral arteries and the middle cerebral arteries. Medially, the paired anterior cerebral arteries are joined by the anterior communicating artery, while posteriorly each posterior cerebral artery connects to the ipsilateral internal carotid via the posterior communicating artery to complete the Circle of Willis anastomosis. Superior to this vascular ring, the inferior surfaces of the frontal and temporal lobes are implied by the surrounding cerebrum, and inferiorly the brainstem forms a midline landmark for the vertebrobasilar inflow. Short perforators are suggested near the circle. Small vessels matter. Continuity between the posterior cerebral and middle cerebral arteries is more than a diagrammatic convenience, it is the collateral logic clinicians rely on when a carotid stenosis or embolus compromises anterior circulation and the posterior communicating artery becomes a pressure dependent conduit. This view supports teaching of aneurysm predilection sites at the anterior communicating artery and the internal carotid–posterior communicating junction, where subarachnoid hemorrhage often presents with thunderclap headache and, in the latter case, an ipsilateral oculomotor nerve palsy. Variants such as fetal origin of the posterior cerebral artery also become intuitive when you can trace the relative caliber and course of the communicating segments. Neuroanatomy and neuroradiology courses can pair this artwork with axial CTA or MRA cases to orient learners to the suprasellar cistern and major branching patterns before moving to cross sectional interpretation. It also fits well in stroke service education materials and neurosurgical publishing on aneurysm clipping or endovascular coiling, where clear identification of A1, ACom, PCom, PCA, and MCA relationships reduces ambiguity in operative planning figures.
