Description
Centered within the male laryngeal cavity, the vestibular ligament (ligamentum vestibulare) forms the fibrous core of the vestibular fold (false vocal cord) and lies superior to the vocal ligament (ligamentum vocale) that stiffens the true vocal fold. Medially, each vestibular fold projects toward the airway, bounding the laryngeal vestibule above and helping define the laryngeal ventricle (ventriculus laryngis) as a recess between false and true folds. Laterally, the ligament blends with the quadrangular membrane as it spans from the epiglottic cartilage toward the arytenoid region, while inferiorly the conus elasticus relates to the vocal ligament. Cartilaginous landmarks typically framing this view include the thyroid cartilage anteriorly, the cricoid cartilage inferiorly, and the paired arytenoid cartilages posteriorly. This relationship matters when you need to separate supraglottic from glottic disease and function. The vestibular fold participates in airway protection and can approximate during swallowing or compensatory phonation, but it does not produce the fine mucosal wave characteristic of the true vocal fold. For laryngoscopy and microlaryngeal surgery, the ventricle and the plane between vestibular and vocal folds guide lesion localization and resection strategy, and edema of the supraglottis (including false fold swelling) can narrow the inlet and contribute to postoperative or infectious airway compromise. Small difference, big implications.
