Description
Seen from the dorsal aspect under the superficial fascia, the four dorsal interossei occupy the intermetatarsal spaces between the 1st through 5th metatarsals, their bipennate bellies arising from adjacent metatarsal shafts and converging toward the bases of the proximal phalanges. Tendinous slips course distally toward the dorsal digital expansions, aligning with the metatarsophalangeal joints and the extensor apparatus over digits 2 through 4. Medially, the 1st interosseous lies lateral to the 1st metatarsal and abductor hallucis (which is plantar-medial in position but often referenced when orienting the hallux), while laterally the 4th interosseous sits adjacent to the 5th metatarsal. Phalanges and tarsal bones anchor the field proximally and distally. Clean anatomy. Dorsal interossei matter because they define the transverse and longitudinal stability of the forefoot while providing abduction away from the second toe axis and assisting metatarsophalangeal flexion with interphalangeal extension through the extensor hood. This relationship is central when teaching claw toe and hammer toe mechanics, where intrinsic weakness shifts balance toward long extensor and flexor tendons and alters pressure under the metatarsal heads. The same anatomy guides intermetatarsal surgical corridors, since the deep plantar arterial arch and intermetatarsal neurovascular bundles lie plantar to these spaces while dorsal approaches must respect the extensor tendons and dorsal cutaneous nerves. Use this dorsal perspective in gross anatomy and functional anatomy courses to correlate intrinsic muscle compartments with metatarsal alignment, or in podiatric and orthopedic teaching to explain forefoot pain patterns, metatarsalgia, and postoperative rehabilitation after hallux valgus or toe deformity correction.
