Description
Arising from the anterior maxilla near the incisive fossa, the depressor septi nasi courses superiorly to insert into the nasal septum and posterior aspect of the alar region, positioned deep to the upper lip soft tissues and just anterior to the anterior nasal spine. From this lateral perspective in an adult male, its fibers sit inferior to the nasal cartilages and superior to the orbicularis oris, blending with the upper lip musculature and lying close to the philtrum and columella. Adjacent bony landmarks include the nasal bones, maxilla, and anterior mandible, with the orbit and external acoustic meatus providing additional craniofacial orientation. Cervical alignment through the atlas and axis is included posteriorly for spatial reference. Depressor septi nasi anatomy matters because its contraction can pull the nasal tip inferiorly and influence alar base position during smiling, a common aesthetic complaint and a recurring surgical planning point in rhinoplasty. Small differences in its origin and depth relative to the orbicularis oris can determine whether tip ptosis is primarily muscular versus cartilaginous, and that distinction affects decisions such as muscle release, excision, or repositioning through an open or endonasal approach.
