Description
Exposed beneath the dermis in an anterior facial dissection, the depressor septi nasi is seen arising from the maxilla in the midline above the incisor region and coursing superiorly toward the nasal septum and medial crura of the alar cartilages. Inferiorly, its fibers blend with the superior margin of the orbicularis oris and interdigitate with adjacent upper lip musculature near the philtrum. Lateral to this midline slip, the levator labii superioris alaeque nasi (highlighted) runs superior to the upper lip toward the frontal process of the maxilla, framing the nasal ala and helping define the nasolabial region. Bony landmarks of the anterior skull, including the anterior nasal aperture, maxilla, and dental arcade, orient the muscle’s attachments. Surgeons and injectors care about this small muscle because it directly influences dynamic nasal tip position and upper lip movement. Overactivity of the depressor septi nasi can contribute to tip depression and widening of the nostrils on smiling, and it is commonly addressed during functional or aesthetic rhinoplasty as well as in targeted botulinum toxin treatments. Its close relationship to orbicularis oris also explains why interventions in the upper lip can unintentionally alter nasal tip dynamics. Small muscle. Big consequences.
