Description
Beneath the superficial soft tissues of the male nose, the depressor septi nasi appears as a short, narrow slip arising from the maxilla near the incisive fossa and anterior nasal spine, then coursing superiorly to blend with the caudal nasal septum and adjacent alar cartilage at the columellar base. In lateral view, its fibers sit deep to the skin of the nasal tip and alar base, inferior to the nasal bones and anterior to the nasal cavity, with the upper lip and orbicularis oris lying inferoanteriorly. Nearby bony landmarks include the maxilla, mandible, zygomatic arch, and nasal bones, with the cervical vertebrae, hyoid, and laryngeal cartilages providing neck context posterior and inferior to the facial skeleton. Small muscle. Big cosmetic impact. During smiling or speech, contraction of the depressor septi nasi can pull the nasal tip inferiorly and shorten the upper lip, a mechanism behind the smile-related nasal tip droop seen in some patients. That functional linkage makes this muscle a frequent target in rhinoplasty planning, where surgeons may release or weaken it through an intranasal or sublabial approach to reduce dynamic tip depression while preserving support of the columella and alar base. It also helps explain postoperative changes in nasal tip rotation and upper lip position when dissection occurs near the anterior nasal spine.
