Description
Seen from the lateral aspect beneath the skin, the inferior extensor retinaculum spans the anterolateral ankle as a thickened band of crural fascia, bridging from the lateral calcaneus toward the dorsum of the foot where it divides into roots over the tarsals. Deep to the retinaculum, the tendons of fibularis (peroneus) tertius and extensor digitorum longus course anterior to the talus and calcaneus, continuing distally toward the metatarsals and phalanges. Posterior and inferior to this region, the calcaneus and Achilles tendon define the heel contour, while the talus sits superiorly within the ankle mortise formed by the distal tibia and fibula. Lateral visualization of the inferior extensor retinaculum matters because it is the primary restraint preventing bowstringing of the anterior compartment tendons during ankle dorsiflexion, and its attachment pattern explains why swelling or scarring after an inversion ankle sprain can produce focal pain over the sinus tarsi and anterolateral ankle. Surgeons use these fibrous roots as landmarks when planning anterolateral ankle arthroscopy portals and during debridement for anterior ankle impingement, where staying superficial to the retinaculum protects extensor tendons and the nearby superficial fibular nerve branches.
