Description
Medially, the inferior extensor retinaculum is seen deep to the skin spanning the anteromedial ankle, with its proximal attachment along the medial tarsus near the sustentaculum tali of the calcaneus and adjacent talar and navicular region. From that medial anchor, the retinacular bands course anteriorly and laterally across the dorsum of the foot, forming fibro-osseous tunnels for the extensor tendons as they pass toward the metatarsals and phalanges. Deep landmarks logically include the distal tibia and fibula forming the ankle mortise around the talus, the calcaneus inferiorly, and the medial malleolar region superior and posterior to the retinaculum. Extensor digitorum longus and extensor hallucis longus tendons lie anterior to the ankle joint, while tibialis anterior passes most medially as it heads toward its insertion on the medial cuneiform and base of the first metatarsal. Orientation matters here because the medial limb of the inferior extensor retinaculum is where failure of the retinacular constraint can translate into extensor tendon bowstringing during dorsiflexion, a recognized postoperative and post-traumatic problem after anteromedial ankle approaches. The same corridor is used in exposure for talar neck fixation and for procedures addressing tarsal tunnel region pathology, where careful soft-tissue handling helps protect the saphenous vein and saphenous nerve running superficially along the medial ankle.
