Description
Across the dorsum of the ankle and foot, the inferior extensor retinaculum appears as a stout, Y-shaped condensation of deep fascia lying superficial to the extensor tendons as it spans the anterior aspect of the tibiotalar region. Its stem anchors laterally near the anterolateral calcaneus, then divides into limbs that course medially, forming fibrous tunnels over the tendons of tibialis anterior, extensor hallucis longus, and extensor digitorum longus as they pass toward the medial cuneiform, first metatarsal, and dorsal digital expansions. Distally, the tendons continue toward the hallux and lesser toes, with underlying metatarsals and phalanges faintly indicated to maintain bony landmarks and tendon trajectory. Skin is removed, so the superficial topography reads clearly. Retinacular anatomy is where extensor tendon mechanics either behave or fail. Loss of restraint or laxity after ankle sprain can allow tendon bowstringing, while post-traumatic scarring or iatrogenic tightness around the retinacular compartments can contribute to painful anterior ankle impingement and tenosynovitis, most often involving extensor digitorum longus or extensor hallucis longus. The lateral attachment and medial limbs also orient you for anterolateral arthroscopy portals and for surgical exposures that cross the anterior ankle, where protecting the extensor tendons and their synovial sheaths matters.
