Description
Beneath the dorsal skin of the male ankle, the superior extensor retinaculum appears as a broad transverse fascial band spanning the distal tibia and fibula just proximal to the talocrural joint. Deep to this strap, the extensor tendons course distally along the anterior ankle toward the dorsum of the foot, including extensor hallucis longus medially and the extensor digitorum longus slips more laterally, with tibialis anterior typically the most medial tendon approaching the medial cuneiform and first metatarsal. The retinaculum sits superficial to these tendons and their synovial sheaths, resisting bowstringing as they cross from the leg to the foot. Clear orientation. Retinacular anatomy at the anterior ankle matters in anterior ankle arthroscopy, open approaches to the talar neck, and any exposure that crosses the extensor compartment. Hypertrophy or scarring around the extensor retinacula can contribute to extensor tendon tenosynovitis, and the nearby deep fibular (peroneal) nerve in the anterior compartment makes this region a common site of postoperative dysesthesia if the fascial planes are not respected. The dorsal perspective also supports teaching of how fascial restraints redirect tendon force vectors during dorsiflexion and toe extension at the metatarsophalangeal joints.
