Superior Extensor Retinaculum, Dorsal View

Arcing transversely across the anterior ankle, the superior extensor retinaculum forms a single, stout fibrous band spanning from the distal tibia medially toward the distal fibula laterally, lying superficial to the extensor tendons as they cross the tibiotalar region. Deep to it, the tendon of tibialis anterior occupies the most medial position, with extensor hallucis longus running just lateral to it and extensor digitorum longus splitting into multiple slips that course distally toward toes two through five. Inferior to this band, the inferior extensor retinaculum appears as the more complex, Y shaped fascial thickening over the dorsum of the foot, continuing the restraint of these tendons as they pass toward the midfoot. This dorsal perspective matters because the superior extensor retinaculum is the proximal checkrein that prevents bowstringing of the dorsiflexor tendons during active dorsiflexion and toe extension, and it sets the roof of the extensor tendon tunnel where friction, tenosynovitis, or post traumatic scarring can limit glide. Orthopedic approaches to the anterior ankle and dorsal midfoot, including exposure for ankle arthroscopy portals, ORIF of distal tibia or fibula, or tendon repair, must respect these fascial bands to avoid postoperative tethering and painful prominence with footwear. It also provides a clean teaching correlate for differentiating tendon courses to the hallux versus the lesser toes at the level where they are most tightly constrained.
