Description
Across the anterior ankle and proximal dorsum of the foot, the inferior extensor retinaculum (retinaculum musculorum extensorum inferius) is shown from a dorsal perspective as a Y shaped fascial thickening with a lateral stem that divides into superior and inferior limbs. The lateral portion anchors to the anterolateral calcaneus near the sinus tarsi, then spans medially toward the medial malleolus and medial plantar fascia, forming osteofibrous tunnels for the extensor tendons. Deep to the retinaculum, the tendons of tibialis anterior, extensor hallucis longus, extensor digitorum longus (with slips toward digits II to V), and fibularis tertius course distally over the talus and dorsal midfoot, held close to the joint line to prevent bowstringing during dorsiflexion. Short dorsal extensors may be suggested on the lateral dorsum, but the retinacular limbs remain the key landmark. Dorsal ankle approaches live and die by retinacular anatomy because the extensor tendons and the anterior tibial neurovascular bundle travel in a tight corridor between the malleoli. Injury or surgical release of the inferior extensor retinaculum can permit tendon subluxation and painful snapping, and scar or postoperative tethering may mimic anterior ankle impingement when the foot moves into forced dorsiflexion. It is also the structure you preserve or repair when reconstructing the extensor tendon sheath region after lacerations across the dorsum of the foot.
