Description
Seen from the lateral aspect of the male ankle, the inferior extensor retinaculum (retinaculum musculorum extensorum inferius) spans the anterior dorsum of the hindfoot and anchors firmly to the superolateral calcaneus at the sinus tarsi region. Its bands pass anterior to the talus and neck of the calcaneus, creating fibro-osseous tunnels that hold the extensor tendons close to the ankle and subtalar joints. Deep to the retinaculum, the tendons of extensor digitorum longus and extensor hallucis longus track distally toward the dorsum of the foot, while tibialis anterior lies more anteromedial and may be only partially seen from a true lateral projection. Bony landmarks such as the lateral malleolus (fibula), talus, and calcaneus define the relationship between retinacula, joint lines, and tendon trajectories. Functionally, this lateral view matters because the inferior extensor retinaculum is the primary restraint to extensor tendon bowstringing during ankle dorsiflexion, and it also provides a surgical and arthroscopic landmark for the sinus tarsi and lateral subtalar approach. Painful snapping of the extensor digitorum longus tendons after ankle sprain, or irritation related to sinus tarsi syndrome, is often understood only when you appreciate how tightly these tendons are tethered against the calcaneus and talus. Small structure, real consequences.
